At Next Journey Orthopaedics, we understand the importance of having easy access to your medical records. Whether you need them for personal use, to provide to another healthcare provider, or for insurance purposes, we are here to help. Below, you'll find the simple steps to request your medical records.

How to Request Your Medical Records

Please follow the direction to avoid to your delays in obtaining medical records.  There are two ways​​

Option 1

  1. Log into your Privia Patient Portal.  

  2. Select "My Health" (located on the left side of the screen, fourth selection from the top). 

  3. Select "Health Records" (located at the top of the screen under My Health, fifth selection from the left).

  4. Select the desired date or date range.

  5. Select the method on how you will get your data.

  6. Please text 571-425-5393 if you have questions.

Option 2

  1. Make sure you have all the necessary information, including name, date of birth, contact information, and specific records you need.

  2. Submit Your Request

    • You can submit via the email

  3. Processing Time

    • Requests are processed within 7-10 business days. We will notify you once your records are ready for pick-up or have been sent to your designated recipient.

  4. Fees

    • There may be a nominal fee for copying and mailing your records. We will inform you of any applicable charges before processing your request.

  5. Authorization: If you are requesting records on behalf of someone else, you must provide legal documentation proving your authority to access their medical records.​

Please provide the recipient's contact information in the email request, and we will send your records directly to them.
